The Nazareth College Athletic Department provides an insurance policy that works in conjunction with your personal medical insurance coverage. The Athletic Insurance policy covers any expenses not covered by your personal medical insurance resulting from accidental bodily injury that occurs as a direct result of your participation in intercollegiate athletics. In other words, the Nazareth College policy covers only expenses that your personal medical insurance coverage does not pay, such as co-pays for office visits or prescription drugs, and partial payments to a health care provider. Covered injuries must occur as a direct result of your participatioin in varsity or junior varsity sports at Nazareth.
If a student-athlete practices or competes with a pre-existing medical condition, any further injury that occurs will not be covered under our insurance policy. Additionally, failure by a student-athlete, parent(s), or guardian(s) to follow the terms and conditions of the student-athlete's personal medical insurance to utilize care properly will not, in any way, increase the liability of the college under this policy. This includes failure to obtain medical referrals or pre-authorizations required for the provision of healthcare at off-campus providers.
In the event you are injured while participating in our intercollegiate program, in practice or in competition, the procedure below must be followed:
- Immediately notify an Athletic Trainer of the injury. The phone number is (585) 389-2838. After injury evaluation, you may be referred to a specialized physician or to an emergency department.
- Submit the bill to your parents' or guardians' insurance company or get proper preauthorization from your primary carrier if necessary under your parents' or guardians' policy.
- In the event your parents' or guardians' insurance company does not cover the entire bill, complete a claim form obtained from the athletic training staff. An itemized bill along with an explanation of benefits covered must also be submitted to the Athletic Training Staff for filing the claim to the Nazareth College Insurance Company.
Failure to report any injury(ies) may jeopardize your coverage under the college's insurance plan. If any injury goes unreported or bills are not submitted in a timely manner, the college will submit the claim, but cannot be responsible for lack of timely filing. Consequently, the student will be responsible for the charges.
Once this procedure has been followed, any balance of expenses not paid by your parents' or guardians' insurance carrier, still remaining, will be submitted to the college's insurance carrier.
